Are You Eligible?

How Does It Work?
- Coverage runs from August 1 to July 31 each year.
- A deductible is calculated at approximately 4% of your household’s net income.
- This deductible is split into four quarterly payments.

- You will only pay $2.00 per prescription until the quarter ends.
- If you apply before September 30, you receive benefits for the full program year.
- Applications submitted later will have a deductible adjusted based on the application date.
What Is Covered?
